On March 28, 1979, the United States experienced a major nuclear accident. In the early morning hours of that Wednesday, two water pumps in the cooling system at Three Mile Island (TMI), a large nuclear power plant in the small northeastern city of Harrisburg, Pennsylvania, simply stopped operating. Although the nuclear reactor's automatic safety systems responded appropriately to this minor malfunction, TMI plant operators did not. A series of errors in the automated operating system of the nuclear power plant and on the part of the plant staff resulted in the worst nuclear accident in United States history.
